Message from Gard and Grand Avenue School Nurse for the 21-22 School Year
Mandatory requirements for 2021-2022 school year:
Students may not attend school until all up to date physicals and immunizations are fully completed and received by the first day of school.
Requirements are:
Pre-K - physical, immunizations and lead screen
Kindergarten – physical, immunizations, lead screen, dental and eye exams
2 nd grade – dental exam
Per school policy, no prescription or non-prescription medication will be given at school without a signed doctor’s order form and signed consent from the parent.
